Some of the documents have tight formatting. Customization of these documents (Student Emergency Contact Card, Parents' Wallet Card and the Student Release Model Guide), even minor changes in text, may disable the layout.
Certain situations may involve releasing students from school or relocating them at a time when parents expect their children to be at the school site. Such actions are authorized by the superintendent or principal only in times of extreme emergency. It is preferred that students remain at school during the academic day.
The documents below were developed by a working group of school administrators, staff and parents to facilitate student release and standardize procedures among Marin County schools.
FOUR STEPS TO STUDENT RELEASE
- PARENTS
- Fill out Student Request Form.
- Show photo ID at REQUEST GATE.
- Move to RELEASE GATE to wait for student.
- Leave campus immediately after student is released to your custody.
- SCHOOL STAFF
- At REQUEST GATE, verify parent ID and authorization.
- Direct parent to RELEASE GATE; radio or send runner for student.
- At RELEASE GATE, verify Student Request Form is signed.
- Release student to custody of authorized adult.

Student Emergency Contact Card - (PDF) (MSWord) (Español)
- The contact card has student and parent/guardian contact information, adults authorized for student pick up and medical background about the student. Print double-sided in LANDSCAPE page setup on card stock.
Student Request Form - (PDF) (MSWord) (Español)
- This form is used to request the release of a student from the school site. The layout is two forms per page. Print on brightly colored paper and distribute to parents to implement the student release process.

- A double-sided flipchart describing the student release plan, including staff assignments, set-up, population assessment, advice for parents, and customizing instructions.
Student Release Advice for Parents - (PDF) (MSWord) (Español)
- A baker's dozen of reminders for parents or guardians when the student release procedures are deployed at a school.
Parents' Wallet Card - (PDF) (MSWord) (Español)
- A pocket-sized card to serve as a reminder to parents/guardians regarding procedures and designated adults authorized for student pick-up. Print double-sided on card stock in LANDSCAPE page setup, then make three cuts to create six cards per page.

- A page of questions which were asked during a Model Student Release Plan workshop to help schools organize their procedures.
Population Assessment Signs - (MSWord)
- Used to inform the incident commander whether all students and staff are accounted for or if there is a problem (missing students, injuries, gas odor, building damage, etc. Print "All Clear" on green sheets, "Need Help" on red sheets in LANDSCAPE page setup and laminate the signs for strength. Distribute to teachers for their classroom emergency backpacks.
72-Hour Medical Authorization - (PDF)
- This form should be provided with a Ziploc bag to store emergency medicine. Print on lime-green paper in LANDSCAPE page setup. Print the Spanish version of the Medical Authorization on the reverse side. A cover letter to parents explaining the request for a 72-hour supply of medicine is available in English and in Spanish. This medication is only used for emergency situations. Medication that must be administered daily to a student should sent to school separately. Unused medication is returned to families at the end of the school year.
